We are passionate about supporting young people with disabilities to achieve their full potential and be a valued and contributory member of their local community.

We are an expanding team, focused solely on working in partnership with education, health, the young person and their families/carers to co-produce innovative and creative circles of support that enable the young person to achieve their aspirations. Our work focuses on Preparing for Adulthood outcomes – employment, friends, relationships and community, independent living and good health.

Fulfilling our remit under the Care Act 2014 and the SEND reforms, we work with young people aged 14-25 to begin planning for adulthood by supporting the young person through the transition from children’s to adult services and from school to college. We remain involved with the young person until their plans for life after school or college have been implemented. By attending the young person’s education, health and care plan (EHCP) reviews, we aim to ensure that all appropriate professionals are involved to facilitate a smooth and seamless transition.

The disability transitions team sits within the adult social care disabilities service, which includes our adult learning disability team, and our physical disabilities and sensory team, so there is a wealth of knowledge and experience within the service. We have a dedicated and committed team, with supportive managers to enable you to develop excellent practice, knowledge and skills, whilst supporting you to progress.

We are based at Saltisford Office Park, in the heart of historic Warwick. We also have office bases all around the county and actively support hot-desking in any of our offices preventing unnecessary travel which is great for the environment and makes the best use of time. We also provide technology to support home-working.
