Early Support and Family Help Assessment
An assessment helps to agree what help and support the family might need (if any) and who is best to give it. The depth of the assessment will be determined by the level of need or concerns. For all assessments certain core information is gathered through a process known as Pathway to Change. If more detailed information is required, this is expanded into a Family Help Assessment.
The assessment and review process has been designed so that professionals can work with families to understand their situation, recognise their strengths and identify actions that will enable them to overcome challenges they are facing.
