Supporting documents

As well as evidence of your disability, you need to provide evidence of your date of birth and your address.

Date of birth - you will need to provide one of the following as evidence:

  • passport (valid or expired) provided it is in the same name
  • driving licence (provided DoB is clearly shown)
  • birth certificate in current name
  • birth certificate plus proof of name change
  • NHS letter or prescription which shows DoB (provided NHS documents are not used for the address)
  • Non-UK National Identity Card
  • marriage/civil partnership certificate if DoB is shown

Address - you will need to provide one of the following as evidence:

Ideally this should be your Council Tax bill for the current tax year.

If you are not named on the Council Tax bill, then send a copy of one of the following, dated within the last 12 months:

  • utility bill in your name (gas, electricity, water) - dated within the last 12 months
  • personal landline telephone bill
  • rent receipt/tenancy agreement - from the council, housing association or letting agency - dated within the last 12 months
  • NHS letter or prescription which shows address (provided NHS documents are not used for DoB) - dated within the last 12 months
  • letter from DWP which includes National Insurance Number - dated within the last 12 months
  • housing benefit/council tax letter benefit - property address matches postal address - dated within the last 12 months
  • personal tax letter from HMRC which includes National Insurance Number - dated within the last 12 months
  • letter from The Pension Service - dated within the last 12 months
  • letter from The Probation Service - dated within the last 12 months
  • letter from JobcentrePlus - dated within the last 12 months
  • current mooring licence - dated within the last 12 months

Please note that we do not accept bank statements or driving licence as address evidence, because this does not prove a residential address. We do not accept poll cards or electoral roll because someone can be registered to vote at more than one address.

Apply online

You can apply online using a computer, tablet or smartphone.

You will need:

  • evidence of your date of birth
  • recent evidence of your address
  • evidence of your qualifying disability
  • a webcam or built-in camera to take your photo or a passport-style photo which you can upload.

Please note that you will currently need to upload your evidence as a PDF file. You can either use a scanner or use an app on a smartphone or tablet which will create a PDF file using your camera.

If you can't finish your application in one go, then you can log back in to finish it later.

Apply by post

Download our Application for concessionary travel pass – disabled person’s pass (PDF, 744 KB) and return it to us by post.

You will need to send us photocopies of your evidence as well as a passport photo.
