The Learning Disability Service predominantly works with customers aged 18-65 years with a learning disability. We also work with those over 65 years of age if they have been known to our service area previously.

We provide assessment, support, review, and advice and signpost to other forms of support. We aim to support adults to maximise independence and autonomy and live full lives within the community.

Physical Disability and Sensory Services (PDSS) countywide 

The PDSS Service works with customers aged 18-65 years predominantly with a physical disability and/or sensory needs. We provide assessment, support, review and advice and signpost to other forms of support. We aim to support adults to maximise independence and autonomy and live full lives within the community.

Disability Reviewing

The Disability Reviewing team supports the Learning Disability and Physical Disability and Sensory Services teams to complete statutory Care Act reviews for people receiving care and support to meet their eligible needs under the Care Act. 

Independent Living Team (ILT)

The Independent Living Team supports all teams within Social Care and Support with information and advice and the administration of Direct Payments. The team support the development of specialised knowledge and professional skills to empower customers to make informed choices, whilst maintaining cost effective solutions.

The team acts as the central point of contact and provides an effective interface with social care teams, Direct Payment customers and external Direct Payment support services. We advise on and provide cross cutting complex information, guidance and support within this specialised area of legislation.
