Assessment Identification and Monitoring
Teachers to consider the following:
- Is recent assessment information used to gain a better understanding of an individual young person’s physical and/or medical needs and to identify whether they are a barrier to their learning in the classroom?
- Are there physical and/or medical conditions, and any treatments the young person has received, and the implications, included in their One page profile?
- Are there any targets/outcomes that young people with physical and/or medical needs working towards?
- Are any changes in physical and/or medical needs recognised and is further advice gained when appropriate?
- Are young people involved in the development and evaluation of any physical and/or medical support strategies in class?
