Wildflower verge application form (DOC, 165KB)
What documents need to be provided with the application forms?
There are three documents that must be provided with the application form before we can progress your application further for consideration. These are:
- Evidence of Public and Employers Liability Insurance showing a minimum £10 million cover for any one claim unlimited during any one period of insurance.
- A sketch or plan, with dimensions and photographs, showing the location of proposed wildflower verge/change of grass cutting regime. An example plan is available to view by clicking the link on this web page.
- A copy of a site-specific risk assessment. An example risk assessment is available to view by clicking the link on this web page.
Additionally, if fire hydrants are positioned in the verge that you wish to turn into a wildflower area, you need to contact Warwickshire Fire and Rescue Service (WFRS) for comment. A copy of the WFRS response, regarding maintenance around fire hydrants will need to be included with your application.
Maintaining highway safety will be the overriding priority. Applicants are responsible for meeting all health and safety requirements, including ensuring anyone working on the verge has the required personal protective equipment and has been provided with the site-specific risk assessment prior to commencing work. An example risk assessment has been provided for your reference; however, it will be your responsibility to carry out your own site-specific risk assessment and include a copy with your application. The Health and Safety Executive website provides risk assessment templates and examples.
