See, Hear, Act - The role we all play
While the Quality Assurance and Improvement Team will lead and co-ordinate quality assurance activity, everyone in the community has a role to play in ensuring that vulnerable people get the best experience possible of the care and support they receive. We want you to tell us what you see and hear when you encounter any care and support being delivered - be it positive or negative comments. If you act, we can too!
We encourage feedback on the quality of care and support by using our “See & Hear checklist”. The checklist provides a set of yes/no questions with an option to add extra information. There is also an additional box at the end of the form for any overall comments if there is something more specific to raise. These checklists help us to gather key information about the quality of a service which then helps us plan our quality assurance activity.
